New Patient Paperwork
Your New Patient Paperwork will be sent to you via email or text once you schedule your initial appointment. This paperwork is completed digitally via the link that arrives in your email. If you require a printed version, our staff is more than happy to accomodate.
Insurance Patients: Paperwork must be completed 3 business days prior to your appointment
Self-Pay Patients: Paperwork must be completed 24 hours prior to your first appointment

We accept most major credit cards and insurance plans as well as self-pay options. To help you make the most informed decisions, we'll discuss the cost of treatment before you begin and help you decide the best route to take.
